- Request Letter
- Copy of FIR including e-FIR/ police complaint/ court injunction order/ copy of plaint (where the suit filed has been accepted by the Court and Suit No. has been given), necessarily having details of the securities, folio number, distinctive number range and certificate numbers, if the market value of securities is more than Rs.10,00,000
- Affidavit cum Indemnity as per format prescribed by SEBI, duly executed and notarized
- Self-attested copy of ID and Address Proof of the holder
- Duly filled and signed ISR - 4
- Client Master List of the demat account of the holder
